Northern Illinois Orthopedics wants your visit to the office to be as smooth as possible. If you are new to the office, or have not seen any of our physicians in the last year, please fill out the patient forms prior to your visit (Form links in left hand column). This will save time when you arrive for your appointment and allow you to give thoughtful answers to the questions.

Northern Illinois Orthopedics participates in most insurance plans, including Medicare. Some of our participating plans include:

  • HMO of Illinois (MCPO)
  • Humana
  • Medicare
  • Private Health Care Systems
  • United Health Care

If your insurance company is not on the list, please check with your carrier to be sure we participate in your insurance plan.

We will file a claim with your insurance company for your visit, but it is your responsibility to understand your policy and its benefits. You must provide a current insurance card when you arrive for your appointment. Depending on your coverage, you will be responsible for any co-payment at the time of service and other amounts due.

Patients should bring a photo ID (driver's license, state ID or Military ID), a current and active insurance card and the Registration Form and the Medical History Form filled out, if possible.

If your case is covered by Workers' Compensation, you will need to bring the claim number, the adjuster's name, the billing address and the phone number.

Your insurance coverage may not provide full payment of the costs for your visit. If your insurance policy requires a co-payment this is expected at the time of your visit.

If you are scheduled for any diagnostic testing or surgery, we will pre-authorize the procedure. However, you might want to check with your insurance carrier to determine your financial responsibility for the procedure.
